Assessments

Assessment services are provided prior to intervention or rehabilitative therapy.

Most assessments are on an individual basis with the client and parent or caregiver, with a single professional based on reason for referral. An initial assessment report may include:
  • Referral source and reason for referral
  • Relevant background information
  • Informal and formal tools used to assess the client
  • Objective findings
  • Clinical impressions
  • Diagnosis(es)
  • Summary
  • Recommendations
  • Plan
A team assessment may be recommended to ensure access to medical consultation for children referred with multidisciplinary needs. A team assessment will provide a multi-disciplinary assessment utilizing some or all of the following professionals:  
  • Speech Language Pathology
  • Occupational Therapy
  • Physiotherapy
  • Social Work
  • and Physician Consultation
 
This approach provides recommendations for service to clients being referred for more than one therapy.
