Referrals - Waterloo Region

In Waterloo Region and Guelph-Wellington, parents/legal guardians or others with the consent of the parent (e.g. doctors, specialists, other service providers, etc.) may make an inquiry or referral to KidsAbility by calling 519-886-8886 x1214 or our toll free number 1-888-372-2259 x1214.

  • Referrals are accepted by the Coordinator of Health Records or designate at the Waterloo site
  • Referrals are accepted when a child appears to meet the age, diagnostic and geographic eligibility criteria for one or more of the services offered by KidsAbility

PLEASE NOTE the following list of EXCEPTIONS:

  • Referrals to the Guelph-Wellington program for children under 6 years are done through the Growing Great Kids KIDS LINE in Guelph, at t 519-846-2715 or 1-800-265-7293 ext. 3616 or by email to info@growinggreatkidsguelph-wellington.com
  • Referrals for Autism Intervention Services must be made to Erinoak Kids in Mississauga; 1-877-374-6625
  • Referrals for Infant Hearing Screening must be made to Erinoak Kids in Mississauga; 1-877-374-6625
  • Referrals to the Guelph-Wellington Inclusion Support Services (accessing support in licensed child care) must be made by calling the KidsAbility Guelph-Wellington Program at 519-780-0186
  • Referrals to the SPOT program in Waterloo Region, (accessing support in licensed child care) must be made by calling the SNAP (Special Needs Access Point) at 519-741-0076

More information about the referral process

  • Referrals can be made by the parent/legal guardian or by a physician or other involved professional, with parental knowledge and consent
  • If the child is not eligible for the service being requested, the Coordinator of Health Records or designate provides information about alternative services
  • The Coordinator of Health Records or designate gathers appropriate information to complete the referral form
  • Client information is entered into the appropriate database(s)
  • The referral form is then processed according to the procedure for the specific service. A Program Manager’s signature is required for approval to some services
  • A copy of the referral is made and distributed to each service provider who requires it
  • The original, hand-written referral form is placed in the child’s main file (Health Record)
  • KidsAbility makes every effort to contact the parent between four weeks and six months after the referral is received. Once contact is made and until an assessment takes place, the parent has the name and phone number of a contact person at KidsAbility, should more information or support be needed.
