Transition to Adulthood Clinic

Have you started to think about your life after high school? Do you have questions about services for adults with disabilities in the community? Would you like to find out about resources that are available to help you with your transition planning?

The Transition to Adulthood Clinic is an opportunity to meet through a Zoom meeting with the Transitions Lead who will help to guide you in the right direction with planning for your future.

What to Expect:

The Transitions Lead will meet with you and/or your parents to provide information and resources that are specific to your goals and plans for life after high school. Prior to the meeting, you will receive an email with some questions to help you and the Transitions Lead prepare for the meeting. A follow up email will be sent to you with links to any online resources discussed. Appointments are up to 1 hour in length.

The clinic is offered monthly throughout the year through Zoom.
